Milton (Nicolas Cage) lived his life as a brutal felon and was deservedly thrown down into Hell for his numerous crimes. He had accepted his fate and serving his time, but when he learns that cultists have murdered his daughter and kidnapped his infant granddaughter to sacrifice in three days time in order to raise the kingdom of Hell on earth, Milton see this as his last chance for redemption for his crimes. Enraged at the prospect that everyone would be doomed to live in misery, Milton breaks out of Hell with the vendetta of rescuing his grandchild and preventing the cultists from accomplishing their sinister goal.
Once back on Earth, Milton begins his pursuit of locating the cult headquarters and meets Piper (Amber Heard), a hard-nosed waitress who is definitely no angel, but joins forces with him and supports the plan by giving him a fire-red muscle car. Meanwhile, Satan has sent his henchman, another ruthless killer known as “The Accountant” (William Fichtner) to track Milton down and bring him back to Hell.
Milton and Piper head for the locations of the people who can give them the information they need to find the headquarters of the cult leaders and find themselves being pursued by the police as Milton metes out revenge on everyone who is responsible for his daughter’s death. At the same time, the Accountant is busy recruiting help to find Milton, apparently with no regard to Milton rescuing his granddaughter.
With the dead bodies of his enemies lining the highway to the door of the cult, word begins to spread that Milton is on the warpath. With only a few hours left before his granddaughter is killed, Milton and Piper pull out all of the stops in their race against time. Meanwhile the cult leaders begin to prepare to stop Milton so that the sacrifice and take place as scheduled. Just before Milton and Piper reach their destination, the Accountant manages to locate them, but is unprepared for Milton’s response to his orders to return to the Underworld.
